Register for a parish activity

If you would like to register for any of the activities below, fill in the appropriate form:

To fill in a form,
    right-click on the link
    select 'Save target as', then save to your computer
    open the saved file in Word, complete it, then email a copy of it to: parish@omiquex.org.uk.

You may be asked at a later date to sign a copy of the form.
